• Course Selection (If you are attending a Summer Registration Day, bring this with you. If you are not, this is due June 7, 2013)
  Ethnicity Form (Due June 7, 2013)
• Student/Parent Information Form (Due as soon as possible and no later than June 7, 2013)
• Health History Form (Due as soon as possible and no later than July 1, 2013)
• Immunization Record (Due as soon as possible and no later than July 1, 2013)
• Pandemic Flu Plan (Due as soon as possible and no later than July 1, 2013)
• Intercollegiate/Varsity Athletics (Due as soon as possible and no later than July 1, 2013)
• Authorization To Release Educational Records (Due as soon as possible and no later than June 7, 2013)
• Summer Registration Day (Due June 7, 2013)
• Wilderness Program Release Form (WOW participants only)
• Campus Store Textbook Preorder Reservation Form (Due Summer Registration Day)
• Knight Card Application Form (Due upon arrival to campus on Summer Registration Day)
• Release of Billing Information Form (Optional)
