Listed below are a number of forms used for the more common transactions in the Registrar's Office, like changing classes or declaring a new major. The forms cannot be submitted online; you will have to print them, fill them out, and submit them to the Registrar's Office (Founders Hall Room 112 or SMC Box 3).
