Administrative History
The R.h.s.j.. took a very active role in the administration of Fanny Allen Hospital. They served in all capacities and in all departments as they staffed the business offices, in addition to their nursing, dietary, and pastoral work. A Religious Hospitaller served as the chief Administrator of Fanny Allen Hospital Hotel Dieu from 1894 until the hiring of the first lay administrator in 1987. The Sister administrator was responsible for ensuring the hospital operated efficiently and effectively and in accordance with the mission, philosophy and traditions of the R.h.s.j... The bursar assisted the superior in financial and other temporal matters, while other Sisters had the responsibility of all the hospital departments.
Custodial History
The records held by the archives of the Fanny Allen Hospital were, by and large, originally created, used, and maintained by the Religious Hospitallers who served as Administrator and Bursar. The files of the Superior/Administrator and Bursar were not kept intact according to their provenance and the records of all the Sisters who served in these positions have not survived. Because of their limited extent and the overlap in certain functions and areas of responsibility for these offices, the records for both have been integrated.
Scope and Content
This series consists of the business records of the Sisters who operated Fanny Allen Hospital from 1894 until the merger with Medical Center Hospital of Vermont to form Fletcher Allen Health Care in 1995, with some later documents. The Sisters who served as Superiors, Administrators, and in the business offices were responsible for essentially all aspects of the hospital and these records reflect their financial and legal dealings, their relationship with the bishops and with the diocese, their provision of health care and dealing with patient accounts, as well as their role serving on the various boards and committees of the modern hospital.
The series is divided into the following subseries: property records; business office records; patient records; annual reports; by-laws and articles of association; board minutes; accreditation documents; restructuring documents; wills, bequests and legal documents; hospital policies; staff records; clippings and historic material including Fanny Allen and her father Ethan Allen; and photographs and audio-visual material.

Scope and Content
This subseries contains files relating to the property owned and maintained by the Religious Hospitallers in Winooski Park (later known as Winooski and then Colchester). It consists of deeds, legal documentation of the transfer of property to and from the RHSJs, correspondence, property appraisals and reports, and blueprints.
Notes
Blueprints and other documentation relating to the current Fletcher Allen Hospital, Fanny Allen campus are held by the current owners of the property.

This subseries contains files originating in the business office of the Fanny Allen Hospital. They were created primarily by the Bursar, but the series also includes files of the Superior. Included in this subseries are correspondence, transaction records, and receipts with lenders and organizations with whom the Sisters maintained accounts; mortgage and loan documentation; records relating to the operation of the Sisters’ farm; financial ledgers; audited financial statements; and the stamps and official seals of the Fanny Allen Hospital, Fanny Allen Hospital Foundation, and Fanny Allen School of Nursing.

01.03.00 Patient records subseries (1894-1994)
Scope and Content
This subseries contains the records the Hospital maintained on patients from 1894 to 1944. Three types of accounts were kept: statistical registers, patient accounts, and surgical and consultation records.
The statistical registers recorded the personal information of the patients. It includes a Register for Patients, covering the dates 1894 to 1920, which records the religion, name, age, place of residence, date of admission and release, number of days in the hospital, diagnoses, results of treatment and remarks. Also, there are statistical registers which recorded the patient’s name, age, sex, marital status, religion, place of birth, occupation, place of residence, name of parents and/or spouse, and the date admitted. The archives has the registers dating from 1895-1901 and 1905-1922. Finally, there is a Daily Record of the Operations of the Fanny Allen, Hotel Dieu Hospital which details the number of male and female patients in the morning and evening, the number of patients admitted, discharged during the day and as the number of patients that died. This is only a partial register, covering the periods between May to October 1895 and July 1939 to April 1943.
Patient Accounts include admitting and financial registers and billing information. The registers detail the patient’s name, date admitted and discharged, amounts owed and amount and method of payment and often include their place of residence, doctor’s name, and treatment given. The archives holds the records from the years 1894-1941 and records on bills collected from 1937 to 1944.
The last group of records, the surgical or consultation registers, the details of diagnoses and treatments are given for the years 1894 to 1910. In these registers, the name, age, date of admission, diagnosis, operations and other treatments given, outcome, and the name of the doctor performing the treatment are listed. Also included in some entries are the marital status, religion, occupation, nationality, place of residence, and the medical history of the individual and their family. A separate register detailing eye, ear, and throat diseases was maintained between 1897 and 1919.

Scope and Content
This subseries contains the annual reports of the Fanny Allen Hospital covering the years 1894 to 1913, 1947, and 1970 to 1992 (with some years missing). The first reports include the names of the incorporators, the chaplain, and attending, consulting, visiting, house, and specialist doctors and surgeons of the hospital; the rules and regulations of the hospital and school of nurses; lists of graduates of the nursing school; photographs; letters from the superior and others; historical sketches and other information on the hospital and the Sisters and staff; and statistics of the surgical cases by diagnosis, the occupations and nationalities of patients admitted, causes of death, and a condensed statement of detailing numbers of patients, treatments and outcomes, and number of days spent at the hospital. There are two types of later annual reports: the first were written by the Administrator and presented to the Trustees of the hospital and the second was written for the public. Both detail patient statistics and finances, and report on staff, the departments, programs, events, major purchases, organizational structure, and future plans.

This subseries contains the by-laws and articles of association that incorporate and govern the R.h.s.j.. of Vermont’s various institutions and corporations over a one-hundred year period. These legal documents were revised and updated frequently in order to allow the institution to meet new demands and allow the Hospital to achieve new goals. These records define the organization, purpose, guiding principles, and limits and powers of the various bodies. The corporations that are covered in this subseries are the Fanny Allen Hospital, Hotel Dieu; R.h.s.j.. of Vermont; the medical staff of the Hospital; Fanny Allen Health Services, Inc., later the Fanny Allen Corporation; the Fanny Allen Hospital Foundation; and the Chittenden County Community Care Corporation.

01.06.00 Board minutes subseries (1894-1995)
Scope and Content
This subseries contains the minutes of meetings of various boards that governed the Fanny Allen Hospital, from 1894 to 1995 (not inclusive). The boards include the Board of Directors or Trustees, Medical Staff Board, Department Heads, and other committee meetings. Unlike RHSJ institutions in Canada, the Fanny Allen Hospital was governed from the beginning by a Board that consisted of Sisters with members of the clergy and lay people. As set out in the articles of association and the by-laws, the Bishop of Burlington acted as the President of the board. Thomas Rotunda became the first lay chairperson of the Board in 1966. The minutes of the annual and special meetings of the board detail proposed plans of actions, elections and removal of members from the Board; detailed financial accounts of revenues and expenditures; amendments to the by-laws and structure of the organization, and other resolutions. Included with the minutes are copies of presentations made to the Board: Board retreat documents, information on board members as well as a Board of Directors orientation manual.

01.07.00 Accreditation documents subseries (1946, 1968-1994)
Scope and Content
This subseries contains accreditation certificates and other documents stating that the Fanny Allen Hospital met the standards of the issuing body. Several such bodies existed and the approval of which the hospital needed in order to remain open. The bulk of the subseries is made up of documents relating to The Joint Commission on Accreditation of Hospitals (renamed in 1987 the Joint Commission on Accreditation of Healthcare Organizations), which was comprised of the American College of Physicians, American College of Surgeons, American Hospital Association and the American Medical Association. This Commission surveyed the hospital, issued a report which made comments and recommendations, and, if the hospital met the Commission’s standards, would issue an accreditation certificate. The length of accreditation would be determined by the Commission (two or three year periods for Fanny Allen) and at the end of that period another survey would be done. In addition to survey reports and accreditation certificates, the files also include correspondence (both with the JCAH and internal hospital letters regarding compliance), questionnaires, and surveys (including interim self-survey progress report) which contain information about the structure and operation of the hospital. A second group of records are from the American Hospital Association, which issued an annual survey to be filled out by member institutions. This survey gathered information on the Hospital’s financial situation, on human resources, and on hospital services. The State of Vermont State Board of Health License to Operate and correspondence and applications to the Vermont Hospital Association are included in this subseries. Finally, two surveys of Covenant Health Systems, of which Fanny Allen Hospital was a member, are included for the years 1992 and 1993-1994.

Scope and Content
This subseries contains documents relating to the corporate restructuring of Fanny Allen Hospital as well as documents relating to the Hospital’s long range plans and its affiliation with the Covenant Health Systems. Two major periods of restructuring occurred: the first in 1983 and the second in 1993-1994. In 1983 the Hospital was restructured to become two corporate entities: the Fanny Allen Hospital, Hotel Dieu and Fanny Allen Health Services, Inc., later the Fanny Allen Foundation, Inc. The primary reason for this restructuring was to protect assets of the hospital and the Sisters and maximizing reimbursement. Documents regarding the 1983 restructuring include the minutes of the Corporate Restructuring Committee meetings; Board resolution; restated articles of incorporation and by-laws; taxation and legal documents and correspondence with legal council; and a memorandum to the RHSJ General Council. The second period of restructuring occurred in the period between 1992 and 1994 when the Fanny Allen Hospital merged with the Medical Center Hospital of Vermont to become Fletcher Allen Health Care. This subseries consists mainly of wills and bequests of individuals, including Michael Kelly, who left money or property to the Fanny Allen Hospital. Also included are a small number of legal files, including the 1894 Process Verbal and Decree between Bishop Michaud and the R.h.s.j... The legal files included in this series range from lawsuits the hospital has been involved in, disciplinary actions levied on staff members, and correspondence with the Hospital’s attorneys.
