New Student Guide

The New Student Guide will arrive in your mailbox in late May. It’s a book full of information that you’ll need to get started on your four years at Saint Michael’s, including some important forms to complete and return as soon as possible. We’ve made the guide available online in PDF form as well. To ensure your forms are completed properly, please be sure to read the entire guide before filling them out.

  • Health History Form (Due June 29, 2012)

  • Immunization Record (Due June 29, 2012)

  • Pandemic Flu Plan (Due June 29, 2012)

  • Authorization To Release Educational Records (Due June 8, 2012)

  • Summer Registration Day Form (Due June 8, 2012)

  • Wilderness Program Agreement, Release and Risk Form (Due with Summer Registration form if participating in WOW)

  • Bookstore Textbook Preorder Form (Due on Summer Registration Day)

  • Knight Card Application Form (Due on Summer Registration Day)
